At Wave Healthcare, we understand that switching to a new provider can be overwhelming, especially when caring for a child with specialized medical needs. Our goal is to make the process as smooth and stress-free as possible for you and your family. Below is an easy-to-follow guide to help you transition to Wave Healthcare.
How to Transition to Wave Healthcare
Step 1. Call Wave Healthcare at 866-935-9283 to speak to a representative. (Select Option 1)
The call must come from a parent or guardian. Request a Change of Provider. During the call, Wave Healthcare will ask for general information:
a. Your name and address
b. Your child(ren)’s name, address, insurance information.
c. The equipment and supplies you want to switch to Wave Healthcare.
Step 2. Fill out The Change of Provider Form and the Physician List Form

Once filled out, you can email or fax to:
a. Email: [email protected]
b. Fax: 866-492-7762
Complete these forms to officially start the process of switching to Wave Healthcare.
This helps us prepare your services and ensures that we have the correct information for you child’s doctor to keep communication clear.
Step 3. Contact Your Physicians
Let your child’s doctor(s) know that you are switching to Wave Healthcare. Ask them to send us current orders for the equipment your child needs to avoid delays. If you have not had an appointment within the past 6 months with the doctors who need to sign the paperwork, please schedule that appointment as soon as possible.
Step 4. Wave Healthcare Will Reach Out
Once we have all the forms, a Wave Healthcare representative will call you to confirm your child’s needs, or in some cases, schedule a home visit to review the equipment and supplies and ensure everything is in place.

Change of DME Provider Letter Submission
Please complete as many fields as possible for our team to find the best solution for YOU. If you are unable to, please do not worry, as long as we have enough information to contact you and your child’s diagnosis that will help our team.

Timeline for the Transition:
1. Insurance Verification
Wave Healthcare will contact your insurance company to confirm coverage.
2. Physician Paperwork
We’ll reach out to your child’s doctor to get the necessary signatures. This process usually takes 5-7 business days, however depending on the doctor’s response time it could take longer.
3. Insurance Approval
Once we have the signed paperwork, Wave Healthcare will send it to your insurance company for approval. This can take 5-7 business days.
4. Scheduling Equipment Setup
After insurance approval, a respiratory therapist will contact you to schedule the equipment setup.
Service Start Date
When switching DME companies, it’s best to set up services at the beginning of the month to avoid billing issues. We will ensure the previous DME provider stops billing at the end of the last month of service, and Wave Healthcare will begin the next month.
Why This Matters
If both companies bill during the same time, it can lead to denials. We’ll work with your previous DME provider to make sure they stop billing at the right time, ensuring a smooth transition with no gaps in service.
Equipment Set Up
When your equipment is delivered, a respiratory therapist will explain how to use it properly. You will also receive a 24/7 phone number for any questions or concerns. Depending on your child’s equipment needs, the therapist will schedule follow-up visits to ensure everything is working well and make any necessary adjustments
Returning Old DME Company Equipment:
Once you have received your new equipment from Wave Healthcare, your previous DME company will pick up any equipment that has not been purchased. The entire process of switching DME providers usually takes about 4 weeks.
